We are writing to formally bring to your attention important information regarding your continued registration with the practice following your move to an address outside our practice catchment area.
While you have requested to remain registered with this practice and we have agreed to this on an out-of-area basis, there are important limitations associated with this arrangement.
Please note that the practice is not able to provide home visits for out-of-area patients.
In addition, access to certain community and locality-based services may be affected. Services such as district nursing, and other community services normally arranged by GP practices, are commissioned and delivered based on a patient’s home address. As your address falls outside our catchment area, referral to these services may not be possible.
If you require medical advice or assessment, you should continue to contact the practice in the first instance. Where it is not appropriate for you to attend the surgery and you require urgent care closer to home, you may be advised to contact NHS 111, who will direct you to the most appropriate local service, which may include a local GP practice, urgent care centre, minor injuries unit, or Accident & Emergency.
For urgent medical needs outside normal surgery hours (6:30pm–8:30am on weekdays and during weekends), NHS 111 will direct you to the local out-of-hours GP provider.
Continued out-of-area registration is subject to ongoing review. If your circumstances change — for example, if your health needs mean you require more frequent face-to-face care, or if the arrangement is no longer clinically appropriate — the practice may ask you to register with a GP closer to home.
If you wish to ensure full access to local GP and community services, you may wish to consider registering with a GP practice closer to your home address.
