NHS App or Find a GP
Once you have completed your new patient registration online via NHS App, Find a GP, through our practice website, please come into the practice with proof of ID and proof of address dated within 3 months. We ask all patients for this as standard, to help us verify details and reduce the risk of incorrect address or identity information on our records. If you’re unable to provide these documents, please let us know and we can discuss alternatives; this will not prevent you from registering. You will be registered within 1 to 5 working days of receiving your details. If we require any further information before registration, we will contact you directly.

Before filling out a registration form, check you are in our catchment area.

Accepts out of area registrations: If you live outside our catchment area, you may be able to register with us on an out-of-area basis. Out-of-area registration means we are not able to provide home visits or urgent treatment at your home address. NHS 111 will direct you to local services if these are needed. Continued out-of-area registration is subject to ongoing review. If your circumstances change, for example, if your health needs mean you require more frequent face-to-face care, or if the arrangement is no longer clinically appropriate, the practice may ask you to register with a GP closer to home.
Registering with the surgery
To register with the surgery:

- Register with Abbey Road Surgery
- Find a GP
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
