New Telephone System

  • Abbey Road Surgery has moved to Total Online Triage for all patient requests from Tuesday 1 July 2025. This means every request is reviewed first by our team, allowing us to prioritise based on clinical need and guide patients to the most appropriate clinician or service. GP practices across UK are moving to a ‘Modern General Practice’ model to make access to care and treatment easier for patients and help improve patient satisfaction. This is a national NHS project, and all general practices must adopt the system.

  • Our telephone lines are closed between 1pm and 2.30pm, Monday to Friday. The surgery is still open for you to walk in with queries that cannot wait until the phone lines open again at 2.30pm. If you have a life-threatening medical emergency that requires an ambulance, please hang up and call 999 immediately.

Integrated community service

Integrated community services combines both health and social care so that the patient gets the right treatment, at the right time in the right place.

The aim is to enhance the current core community, nursery team (district nurses) Isobel Hospice Team with highly skilled health care professionals to enable patients to stay at home rather than being admitted to hospital (30% of admissions to hospital are avoidable).

We have listened to patients and we want patients to tell their story once, enhance the communication process between the patient and those who are caring for them, so care is delivered in an integrated way.

GP practices will align to the integrated community team to enhance this process. To enable health and social care teams to work more effectively together we will be sharing basic medical information for us to effectively manage patients with multiple long-term conditions.
